The agenda included establishing a Government commission on the Kerch Strait oil spill cleanup; updating the rules for providing state support to the agricultural industry; and discussing the 2025-2027state programme guaranteeing free medical help.

Mikhail Mishustin: Good morning, colleagues.
Last week, the President held a meeting with the Government members. The accidents involving tankers carrying petroleum products in the waters near the Kerch Strait was one of the critical items on the agenda of that meeting.
At the President’s instruction, a Government commission to coordinate the cleanup efforts led by Deputy Prime Minister Vitaly Savelyev has been established. Head of the Emergencies Ministry Alexander Kurenkov was appointed its first deputy. He will also oversee day-to-day operations of all on-site services.
The commission also includes Minister of Natural Resources and Environment Alexander Kozlov, Minister of Transport Roman Starovoit, and Finance Minister, Anton Siluanov, as well as heads of other ministries and agencies, and officials from regional authorities.
Colleagues, it is crucial to restore the unique ecosystem of the Black Sea coast in the Krasnodar Territory and the Republic of Crimea as soon as possible. The areas must be cleaned of contamination, and all necessary measures must be taken to prevent petroleum product leaks.
Mr Kurenkov, please maintain constant monitoring over the progress of these operations.
It is also essential to conduct continuous air quality and water sample monitoring, including drinking water and the Kerch Strait waters.
I have invited Head of Rospotrebnadzor Anna Popova to this meeting to ensure this situation remains under her personal control as well.
Many volunteers and concerned citizens are helping the rescuers in the affected regions. It is crucial to strictly adhere to the existing sanitary standards while working with them.
Mr Kozlov will soon present cleanup proposals to the President, taking into account the upcoming rise in the Black Sea water temperature. We thoroughly discussed this at a meeting with the President.
Mr Starovoit will also work on plans for salvaging the sunken tankers. This issue was addressed during our discussions as well.
And I ask head of the Federal Service for Supervision of Natural Resources Svetlana Rodionova to organize a comprehensive assessment of the environmental damage for compensation in full by the guilty party.
It is important to significantly raise the work efficiency. In doing this we need to use additional financial and human resources and, importantly, to engage science.
Mr Saveliev, I ask you to keep all the issues under personal control and report to me.
Now to another topic: measures taken to solve personnel issues in the agro-industrial complex.
As the President noted, with the growth of technological sophistication of agricultural production we will have to pay special attention to the training of scientific and teaching personnel and young specialists. Moreover, that with every year rural areas provide more and more opportunities for self-fulfillment and career growth, for launching own business and building a house.
We discussed these steps in detail earlier during the strategic session. Given the presidential instructions, the Government has adjusted the specialised State Programme for the Development of Agriculture.
In particular, we have specified the procedure for and conditions of granting subsidies. We have also envisaged reimbursement of the costs under apprenticeship contracts and special-purpose training agreements.
In addition, we will partially compensate for the labour and accommodation costs of students during their internships to make it easier for young people to get used to the company if they want to return there for permanent employment.
Specially demanded scientific personnel and teachers, who are required for implementing key projects in the agro-industrial sector, may be eligible for incentive payments, including for buying a house or an apartment.
The same support – for solving housing problems – will also be offered to the teachers, who give lessons to school students in the agrotechnological classes.
Comfortable conditions must be created for the children receiving such additional training. This is why we have to help in major repairs of such educational institutions and in equipping them as well as secondary specialised educational institutions, primarily those supervised by the Ministry of Agriculture. We have about 40 of them.
In total, the amount of costs reimbursement at the expense of the federal budget will make up 30 to 95 percent. We have earmarked over 6.5 billion roubles for these purposes this year.
The provision of additional support for small rural entrepreneurs serves as an additional measure for improving the situation with human resources. Veterans of and participants in the special military operation, who have been discharged from the Armed Forces and who are ready to work in the agricultural sector, will receive this grant.
Farmers are entitled to these changes from 1 January. I am asking the Ministry of Agriculture to ensure the implementation of presidential objectives to improve the HR situation in the agro-industrial sector. Apart from boosting labour productivity, the implementation of these measures will help create jobs in the countryside.
The next issue deals with healthcare.
The Government has approved a programme of state guarantees for providing free medical treatment over the next two years.
While formulating the document, we heeded the President’s instructions, including those aiming to support participants in the special military operation. They will continue to receive medical treatment on a priority basis. Medical organisations will also collaborate more actively with the Defenders of the Fatherland state foundation.
We will also stipulate more affordable sanatorium and resort vouchers, as well as various innovations for the residents of small remote communities. Doctors and paramedics will be able to organise home care, providing the required medications and instructing patients how to use them. Of course, specialists should monitor this entire process.
Ms Tatiana Golikova, please tell us how the programme will be implemented in the next three years.
Second, as you mentioned, the President has declared 2025 the Year of the Defender of the Fatherland. In this context, the programme now includes a dedicated section focused on providing medical care to demobilised participants of the special military operation. This initiative introduces a proactive approach to delivering primary healthcare to them, detailing specific procedures for medical examinations with an expanded list of specialists involved. This is particularly vital for those who have returned to work after sustaining wounds or injuries during their service. Additionally, the programme ensures that participants requiring continuous monitoring by medical professionals and residing in remote areas can receive such care through telemedicine technologies.
Specialised medical care, including high-tech treatments and medical rehabilitation, will be provided to participants of the special military operation with priority access.
They will also receive precedence for spa treatment services. To facilitate this, regions will identify and designate spa facilities equipped to accommodate these individuals, including those requiring care following prosthetic procedures.
The third change involves establishing a process for transferring discharge summaries of patients who have undergone inpatient treatment to outpatient clinics at their place of residence. This measure is crucial for citizens requiring ongoing treatment, medical rehabilitation, and continuous monitoring in outpatient settings.
The fourth change focuses on spa treatment, with a dedicated section detailing its role in promoting citizens’ health. Notably, as part of the new federal project Health for Everyone under the national project Long and Active Life, citizens now have the right to undergo assessments during their spa treatment. These evaluations aim to identify risk factors for disease development and provide recommendations for maintaining a healthy lifestyle, including guidance on proper nutrition.
Fifth, the procedure for prescribing medications not registered in our country to citizens with severe and life-threatening conditions has been clarified. These prescriptions will be based on the decisions of medical commissions. The Ministry of Health is expected to issue the corresponding guidelines in the near future.
Sixth, as you have already noted, is the concept of home hospitals. While this option existed previously, it was not utilised to its full potential. The procedure has now been clarified, particularly for citizens residing in sparsely populated or remote areas, such as rural communities, where access to the nearest medical facility is challenging. In these instances, medical personnel from feldsher-midwife stations and outpatient clinics are authorised to provide necessary medications and perform medical procedures in the patients’ homes. However, as mentioned, this will be conducted under appropriate supervision.
Finally, the seventh point. As I mentioned earlier, we have increased funding for the state guarantees programme. This increase accounts for a more than 17 percent growth in the wage fund portion of the tariff structure for medical care in 2025. Oversight of the proper allocation of these funds for wages is entrusted to territorial funds and regional health ministries. It is crucial that regional leaders effectively organise the implementation of the newly adopted state guarantees programme, ensuring that all the innovations included in this programme are formally established and put into practice. Naturally, both the Ministry of Health and the Federal Mandatory Medical Insurance Fund will oversee this process to ensure its successful execution.

It is essential to continue taking all necessary measures to improve the quality and accessibility of medical care across the country. I ask you to maintain personal oversight of these issues.
